Transcript of Telephone Conversation between Charles Balis, M.D. and Ms. Angelica Alvidrez respecting Nina Alvidrez, Monday, June 22, 1998 at 4:00 pm.

Dr. Balis: Hello?
Ms. Alvidrez: Hello. Yes, is Doctor Balis in, please?
Dr. Balis: This is he. How may I help you?
Ms. Alvidrez: Hello, Doctor Balis. My name is Angelica Alvidrez. My daughter, Nina, and I work at SII. You were referred to me as the therapist that's covered under our health plan. I'd like to set up an appointment for my daughter.
Dr. Balis: I see. What seems to be the problem?
Ms. Alvidrez: Nina has just started working at SII, and she seems to be having problems acclimating to work in a corporate environment.
Dr. Balis: How so?
Ms. Alvidrez: She is very shy and withdrawn. I spoke with her boss last week to see how she was doing. He told me that Nina is very quiet and that he has a hard time getting her to communicate with him. This is a good opportunity for Nina, and I'd like to do my best to help her excel at SII.
Dr. Balis: Has she always been this shy?
Ms. Alvidrez: She's always been a bit on the timid side, but it's been getting worse lately.
Dr. Balis: When did you notice the problem developing?
Ms. Alvidrez: She used to have a few friends from high school, but she rarely speaks to them anymore.
Dr. Balis: How old is Nina?
Ms. Alvidrez: She's nineteen.
Dr. Balis: Does Nina know that you're calling me, Ms. Alvidrez?
Ms. Alvidrez: Yes, we've talked about it.
Dr. Balis: Okay, I'd be more than happy to meet with Nina. You should understand that our sessions will be confidential and that I will not be able to share with you what happens during my sessions with her.
Ms. Alvidrez: I wouldn't expect you to.
Dr. Balis: Good. Is there anything else I should be aware of?
Ms. Alvidrez: I don't know if it's important or not, but Nina is always complaining about aches and pains. One day, it's her stomach. Another day, it's her foot. Then, it's headaches or toothaches. But she never goes to the doctor. I think it's probably just for attention.
Dr. Balis: I'll make a note of it. Will Nina be calling me to set up the appointment?
Ms. Alvidrez: No, I've been trying to get her to do that for over a month now. I told her I'd set up the first appointment for her.
Dr. Balis: All right, how about this Thursday, the 25th at 10 am?
Ms. Alvidrez: That sounds fine; I'll let Nina know. Thank you, Doctor Balis.
Dr. Balis: You're most welcome, Ms. Alvidrez. I look forward to meeting Nina. Goodbye.
Ms. Alvidrez: Goodbye.
