And for each patient in the future, fill out the forms upon making your initial contact. The forms must be filled out again after every 4th therapy session, should such sessions be deemed necessary. Please return those covering current patients within two weeks As these are triplicate forms, please make sure to press hard so the bottom copy is legible.
